These forms are to be completed if medication is administered at school.

MEDICATIONS AT SCHOOL

Whenever possible students are encouraged to receive medications at home. If medications are required during the school day the VCS school nurse complies with the State of Ohio laws and school policies.

 

Please Note: These forms will be available at Open House and the New Parent Meeting. You can also request a copy from the office.

 

Prescription Medications

Prescription medications may be administered by personnel designated by the school principal in grades K-12. Prior to administration of any prescription medication, the proper forms must be completed.
