Request for Student Absence
 

Parent or Guardian,
 

As stated in the Student/Parent Handbook, Parents/Guardians  who wish to take their student(s) out of school for more than two (2) days must complete the Request for Student Absence Form  at  least seven (7) days prior to the scheduled absence.

The link above will open a Google Form for you to complete.  Once you submit the form, the request will email the principal for approval.  You will also receive the same email (sent to the email address you provide) with the completed document.  It is best to use Google Docs to view this document.  This will be for your records and you will also be able to see if the principal approved it or not.  Please keep in mind that it may take a few days for the principal to respond.

 

The students are responsible to complete all class work which will be assigned during the days of their absence unless teachers have stated  otherwise. Such special arrangements must be documented. It is to be understood that the days of  absence are unexcused and are charged against the maximum ten (10) days for the semester. Such absences do not obligate the teacher to provide makeup tests or quizzes, nor to accept any overdue  reports or assigned work.
